Required Faculty Forms
Complete each of the forms below. You will receive an emailed copy of every submission for your records.
- 1. Faculty Information and Travel Logistics — contact details, professional profile, biography, headshot, and travel dates.
- 2. Presentation Details and Syllabus — titles, learning objectives, abstracts, and slide deck uploads for up to three presentations.
- 3. CME Financial Disclosure and Conflict of Interest Attestation — required of anyone involved with CME content.
- 4. Honorarium Agreement, Financial Policy and W-9 — payment preference, tax documentation, and travel policy acknowledgment.
All four forms include a Save and Continue Later button. Use it at any point and we will email you a link to pick up where you left off.
